Contact your Cellular Network company's Customer Care and they'll help you with this. (To make it more clear I mention some Popular Cellular Service Providers : Vodafone, Airtel, Idea, Tata Docomo, Reliance, Aircel, MTS, BSNL, MTNL etc.). only the network service provider can give u the puk code.
u need to call the customer care from any mobile ,same as of ur network,
talk to customer care executive, he will ask u to give the sim card number, a 16 digit number written on one side of the sim, then the customer care executive will tell u the puk number.
puk code can be called and got from the service provider customer care number... your number seems to be like airtel,please contact airtel customer care..
hey your service provider will have a 10digit customer care number also which you can dial from a landline phone..you can get this number from where you get recharge vouches..call them and they will give you youe puk number else take phone of a friend with same service provider and call the customer care rest you know wat to do..
