You should call your operator and ask for the PUK, there is no other way. PUK is SIM related, so they will know your PUK code. Don't trying guessing it, you will have to take it to service if you type in a wrong PUK!

The PUK code can be found in the packaging of the sim card when you first bought it.
Several wrong tries for a pin code can get the sim card blocked and only the PUK code can enable it for access. If you can't find the PUK code, you have to buy another sim card or if this is postpaid, you have to report it to your Telecom Operator.

Best thing would be to backup (if you have not already) the data on the phone using blackberry desktop software..
