The phone service provider are the only ones that can give you the PUK code.
PUK stands for Personal Unblocking Key. This is a uniqueeight-digit code specific to your SIM card. This is required when the PINnumber to unlock your mobile phone is entered incorrectly 3 times. You are thengiven 10 attempts to enter your PUK code. If an incorrect PUK code is enteredafter the 10th attempt, you will block your SIM card permanently and willrequire a new SIM card from your mobile phone provider.

Your cell phone service provider can give you the PUK code once you give themyour account details and they have confirmed your identity.
If you need the PUK ( Personal Unblocking Key ) for your phone, you will have to contact your mobile phone service provider.
PUK code is related to the SIM card you are using and not the phone. So, kindly call the Customer support of your mobile service provider and they will provide you with the PUK code free of charge.
Also, entering a wrong PUK code ten times in a row disables the SIM card permanently. So, please be careful while entering the PUK code.
