Try to input another source(DVD Player) to the same jack you are using as input for your set top box to determine where the fault came from.
If the symptoms does not appear, this is an indication that the fault came from the set top box itself. You should contact your service provider to check the condition of the box and confirm if it is already faulty and need a replacement.
Let me know if the symptoms still persist even with the DVD as the input.
