SOURCE: front end vibration
It sounds as though your car has two seperate problems. The first problem with the vibration that goes away when you let off the gas pedal is caused by a bad INNER CV joint. It only vibrates when you are accelerating because that is when the joint is under stress. Your vehicle has two axles, each with an inner and an outer joint. My suggestion is to have a competent shop determine which is the bad side and replace the entire axle. The second vibration which occurs at any speed is most likely cause by either a bad tire, or a bent rim.

This must be a case of worn or nearly worn brake pads, happens to me all the time. As you buy new disc pads, make sure you use a 'break in' conditioning to the new pads, meaning, from speed of 30-0 slam the brakes, then cool off, then 45-0, slowly do the same if it is safe to do so, and on a clear road, if possible do a 60-0 do the same thing. Before doing these, I have to say make sure, either have your rotors checked by a mechanic if it needs to be cut to specification meaning there are no ridges/circular deep lines on the rotor surface. I have the same problem but i finally had it fixed.
Your front rim is warped. Have it aligned. Balancing alone would not do the trick as this would only balance the wheel on its vertical axis. (assuming they were able to balance it properly)
Jack the front end up and turn the wheel.. Have a screwdriver or any thing that can serve as a guide with the rim, turn the wheel and if it touches the screw at some point of the turn , then the wheel is misaligned. A new stock rim would cost too much so have it aligned with a good machine shop.

There's also a chance that the fork fluids would cause this. Fluid levels should be equal. (less likely)

SOURCE: 1998 honda crv vibrates when starting out
Increased vibration from a vehicle indicates worn out engine supports. This vibration often disappears when revving the engine high, but is very noticeable at low revs.
You can have these inspected and possibly replaced to reduce the vibration cominf from your engine.

What is the rear tire pressure? If it is good lok at the next weak link in the drive line, check the universal joint. if this has NO play at all then I would most likely point out that the differential is wearing out, when was the last time the dif oil was changed? are you running the proper dif oil? When you have the dif apart are the edges of the gears sharp? if they are then a differential may be in order.
