Only the service provider can give you the proper PUK code. You have to call them and get teh code from them.

My sim card is blocked by my own mistak, so need puk code of my tata indicom number 9224769475 PLS HELP

PUK Code
PUK stands for Personal Unblocking Key. This is a unique eight-digit code specific to your SIM card. This is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. You are then given 10 attempts to enter your PUK code. If an incorrect PUK code is entered after the 10th attempt, you will block your SIM card permanently and will require a new SIM card from your mobile phone provider.
PUK codes can be obtained through the following ways - The PUK code is in your SIM card documentation Starter Pack. If you do not have this then - Please contact your phone service provider.
Your cell phone service provider can give you the PUK code once you give them your account details and they have confirmed your identity. For details to obtain the PUK code for the SIM card, please click on this link and follow my instructions.

What is a PUK and how do I get my phone unblocked?

A PIN Unlock Key (PUK), also known as a Personal Unblocking Code (PUC) is used to reset a Personal Identification Number (PIN) that has been forgotten or lost. Most mobile telephones offer the feature of PIN protection. After switching on the phone, if the PIN security function is active, the user is requested to enter a 4-8 digit PIN enabling the phone's non-emergency calling functions. If the wrong PIN is typed in more than three times, either the SIM card, the device, or both become locked. They can be returned to their original unlocked state by entering a PUK, provided by the service operator after verification. If the wrong PUK is entered ten times in a row, the device will become permanently blocked and unrecoverable, requiring a new SIM card. Cellular phone users are therefore advised by most providers to keep their PUK written down in a safe place separate from the device. The PUK (PUC) is a SIM-specific code assigned by the service provider. It is near-impossible to guess and enter it; thus, if a user does not know his or her PUK and is prompted for it, it should be obtained from the service provider. Once you enter the PUK code, you must set a PIN. The PUK and PUK2 are used to unlock the PIN and PIN2 codes respectively if your SIM card is blocked.

Is a PUK stay i fi get a new phone?

If you use the same sim card, this PUK will still be there. You must contact your provider for the puk code. If you still have "attempts" remaining you can save the sim card. If it says PUK Locked...then u need a new sim card.

Sim Card is now lock. I forget the puk code. The service provider has no puk no. information

You will have to get a new SIM card sorry.
PUKs, or personal unblocking keys, are used to unblock a mobile phone which becomes blocked. Mobile phones may become blocked when a PIN (personal identification number) is entered incorrectly on three consecutive occasions. In some instances, the phone may become blocked, which requires a PUK to unblock it, and in other instances, the SIM (Subscriber Identity Module) card itself becomes blocked and again will require a PUK to unblock it. Mobile phones are also blocked when they are reported stolen and if there is an outstanding payment on the account. It is important to differentiate between unblocking phones (using a PUK) and unlocking mobile phones (using another type of code). NOTE: It is a criminal offence for anyone to attempt to unblock a mobile phone, other than the network provider who put the block on the phone. If your phone has become blocked by mistake, for example you have entered the PIN incorrectly on three consecutive occasions, then you need to contact the mobile phone network operator with your details. To explain a little more, specific to your mobile phone network, You need to go to your Service providers, web site & navigate your way to the specific PUK details for your mobile phone network operator. Usually you have to Register your phone to get a PUK code.

Give me unlock key for imei 35664005162345

There are two type of phone lock:-

1. Sim Lock

2. Phone lock

Sim unlock: To unlock sim ask pin or puk no from your mobile service provider and then enter the no to unlock, if you get the msg as sim rejected then you have to go for a new sim card to activate your number

Phone Unlock: To unlock phone with IMEI no is not easy you must know the phone unlock number and it may differ with user( Nokia common Unlock code is 12345 or 0000), you better go to Nokia service center and update the software to unlock the phone easily and it cost only Rs:400.

Thank you.
