Ack, I just recently got the same problem. I turned off my DS while it was saving settings (on the DS) and now my AR doesn't recognize any codes on it, nor does it delete anything when I delete all of the codes through the manager

I have just found the solution to the problem (2 minutes later XD)
Quote CodeJunkies' Support site:

Try resetting the Action Replay by following these steps:

  1. Insert the Action Replay into the DS
  2. While the Action Replay software is loading, press and hold all the following buttons: A + B + Start + Select.
  3. Release all of the buttons once the Action Replay is at the main menu.
The first screen should persist for much longer than normal, and when the Action Replay DS main menu loads, your hardware should be reset.

Once the reset has been performed, try connecting the USB cable to your PC, opening up the code manager software and transferring data again.

It won't return all of your games back, but it will allow you to start on a clean slate again. (Using the update feature will bring most, if not all of your games back

Ahh I see where you're getting at (the red and blue lines thing, must've missed that) The first Action Replay I purchased had that problem, I returned it promptly to get a new one. Before doing so, try cleaning off the pins on the AR (through an alcohol solution, NOT blowing on it). Try using it then, and next the reset combination, if you still cannot access your Action Replay, it may have been corrupted through a fake code that's been floating around the net. It's a one-liner that's been said to do certain things but it instead corrupts your AR and prevents you from using it. If you do remember entering a code similar to this one:

    1AB3C56DA121D2

Then you may have been duped by a fiendish person on the net. (Not saying you have, just saying you might have)

Again, if you can, and all else fails, replace the Action Replay from where you bought it, if it's within the warranty (I really don't want to say buy a new one)

Try resetting the Action Replay by loading it on the DS and holding the A, B, Start, and Select buttons. The first screen should persist for much longer than normal, and when the Action Replay DS main menu loads, your hardware should be reset. At this time, try connecting it to your PC and transferring data again.

My Action Replay appears to have lost all codes however it will not allow me to resave any new codes to it. It says operation failed, not enough space. It should have plenty of space as all codes have been...

You can fix the device just hold a+b then turn on the ds still holding a+b when it says this product is not endorsed by nintendo just hold start and select now you are holding 4 buttons continue to hold these buttons until the main action replay screen its displayed, then release. Now connect the action replay to your computer and now it will accept every code you want.I hope this was useful for you.

Action replay doesn't let me add codes in custom code list!!!!!

Your Action Replay DS hardware may require resetting if you are experiencing any of the following problems:

* Code Manager PC software is reporting that Action Replay DS does not contain enough free space when it actually contains no codes.

* Code Manager PC software is reporting the console`s battery is low when it’s actually fully charged.

* Multicolored bars are shown on the screen after you confirm the entry of a game ID when trying to manually enter codes.

* You can use the Code Manager PC software without any problems until the Action Replay DS is connected via USB set to the update mode on the console, at which point Windows shows a run time error.

If you are experiencing any of these problems, please follow the steps below to reset the Action Replay DS hardware. Please beware that resetting the Action Replay DS hardware will not restore a default codelist, and any codes which are stored on the Action Replay DS will be erased.

1. With your console powered off insert the Action Replay DS

2. Hold down buttons A+B

3. Turn your console on

4. With A+B still held down, as soon as you see the Action Replay DS license screen press and hold down buttons START+SELECT so that you now have 4 buttons held down

5. Hold all 4 buttons down for around 10 seconds until the Action Replay DS loads to it`s home screen

6. If Action Replay DS doesn’t hold on the license screen for around 10 seconds when you do this, you may need to turn off and try again
