When you enter the wrong PIN-code three times in a row the SIM will be locked. To unlock it you will need to enter a PUK-code. This code usually can be found together with the papers you got when you first got your SIM-card. If you can't find it you can try to contact the operator, e.g ATT and ask them for support. They can usually give you the PUK-code if you give them your personal info.

if u lock your sim card normally when you switch on your mobile it will ask to enter your pin code, if you enter 3 times pi code incorrectly ,then it will ask puk code if you enter puk code 9 times incorectly the sim will get blocked. and sim get rejected.only one solution is there to contact costemer care for applying for new sim with same number
Someone must have activated the PIN lock or the SIM lock. If you have entered the PIN incorrectly for 3 times, you will need the PUK to unlock it. The PUK code can be obtained from your phone provider.
the puk code is part of a sim lock. most phones have two locks a phone lock and a sim lock. the puk code comes from incorrectlly entering the sim unlock code too many times. this can be fixed in about 2 minutes but you need to get the sim puk code from your carrier i.e. att, or tmobile. the when you get the pin you want to enter **05*(enter the puk code you were given)*(your new pin of your choice)*(the pin you choose again)# after that your phone will say unlocked besure to test this by turning your phone of and on when it asks you for the pin it will no be the pin you chose, i hope this helps!!!

locked.And it will ask for PIN code.Both these PUK and PIN code can be given
only by the network service provider. Please call the network service provider

In your case I believe your SIM is now FROZEN or locked. You mostlikely will need to purchase a new SIM card.
PUK code (Pin Unlock Key) is required when you enter an incorrect PIN 3
times. After entering the wrong PIN 3 times, the card will be locked and
you will need to get a PUK code from your service provider to unlock it.
If the wrong PUK code is entered ten
times in a row, the SIM card becomes invalid and you will need to purchase a new SIM Card. The phone will display
