Clean out rotary hook area following manual instructions on how to do this, usually removing bobbin holder and bobbin, then the rotary hook area by undoing the latches each side.

Could be a thread jamming in there causing the error message.

How can I open/clean hook system?

Unable to assist without knowing the make & model. Consult the owner's manual. There should be instructions re how to remove the bobbin to access the hook area. Check the maintenance section.

Baby lock quest

The top thread is looping right? This particular machine loves a well oiled hook system, and it needs to be vacuumed out regularly. Compressed air is not good on this machine, actually it is not good on any machine if the covers are on. So check for thread trapped inside the hook system by flooding with oil and oscillating the machine back and forth for a few minutes.
Two other possibles: something is snagging the top thread like thread trapped inside the hook system or a little burr on the stitch plate. Moving faster means the fabric tractors harder, so any burrs will be mre noticeable.

I think timing is correct because hook catches thread every time, but when the hook finishes its rotation and releases the thread, the thread loop doesnt make it all the way around the bobbin

the catcher/hook on the bobbin case system loosen or not fix properly probably becoz of vibration or something( check or replace new), then check upper system either tension/ tighten of your thread/yarn or correct..

Loose top thread bernina 1008, tension problem but no solutions yet...

Are you sure you are getting the top thread between the tension discs? You should be able to tell the difference when you lower the pressed foot.

My phaff wheel seems to be locked up -- what can i do to fix it the needle will not go down to pull the bobbin thread thru ,and it "sticks " when i try to turn the wheel

There may be a piece of thread logded between the hook and the bobbin case holder. Try turning the handwheel very firmly backward (opposite the sewing direction).
