Needles are normally inserted with the flat side of the top part facing away from you. I've yet to see a machine that it isn't inserted that way in 30+ years of sewing. If, for some reason, the machine doesn't sew or isn't sewing well with the needle inserted that way, you can always flip it around and try the other way. But start this way first. I'm certain it'll be the correct way on your machine as it is on every machine I've ever come across.

How to use a twin needle on my SR2000 toyota sewing machine

First, verify that your machine can use a twin needle. Most machines take will use a twin needle will have separate thread guides and duel thread spool pins.

To thread it, thread one thread all the way through one of the needles and then thread the second thread through to the second needle. This will help keep the threads from twisting.

I have an old machine - a New Home SR2000 - I have been using it for 28 years and I now have a problem with puckering at the back of the material

Looked around but found very little on this machine. In the event a manual for the machine will help, you can purchase one at this URL:
http://www.sewingmanuals.com/Sewing_Machine_Manuals/New_Home.htm
it is item #60 on the list.
You might try Googling under the manufacturer's name and model to see if there is a Users Group.
Sorry I could not be of help. Paul

Black bobbin case

If this is the machine I think it is the bobbin case is completely round as opposed to half round. If it is half round ignore the following.
Remove the presser foot and then the two screws which hold the needle plate. Lift off the needle plate and then lift out the old bobbin case. Clean all the lint out and then insert the bobbin case so that the less round part of it is to the back and the lip around the bobbin case is resting on the lip around the hook. Turn the hand wheel a few times to be sure it is working smoothly and not binding. Replace the Needle plate , bobbin and presser foot. Insert a new needle, thread the machine, Check your tension to be sure it is working correctly, turn the hand wheel again to be sure it is picking up the thread without binding and sew.
