This is usually due to a damaged disc. Take a look at your disc, and see if it is cracked, scratched, smudged, or chipped. If so, you may need to get it cleaned, or buy a new disc. If it is due to scratches, then you can use toothpaste to fix it. just apply a small amount of toothpaste to a soft cloth, and wipe gently from the center of the disc, outwards. You can also clean the disc in a similar manner, if you just do not apply toothpaste.
