Built in speakers on laptops are generally low quality, and you will not get very good sound from them, especially compared to headphones.
The output for headphones is fairly low-level and is obtained before the signal is sent to the final 'power' amplifier. This means that either the power amp has failed or you could having speakers with cones that have separated from their frame causing unwanted vibrations. Check if both built-in speakers are 'rattling' or if it is only occurring in one. If only one, then the speaker has come unglued from the frame, if both, then the (likely main board-mounted) integrated circuit has failed since there are normally multiple channels combined into one package.
If on battery power:
Some laptops make little "squicky" noises sometimes. This is normal for some laptops.
Try re-installing the software for your soundcard using the dell support site: http://support.dell.com/
If on grid power:Please check that you are connected to a grounded outlet. Next you should check if there are no machines like washing machines, microwaves or vacuum cleaners on the same connector box as your laptop is. Those devices can create some disturbance.
