If its asking for your SIM pin code, its 0000 (four zeros). If its asking for your PUK code, you'll have to call your carrier. If its saying PUK blocked, you need a new SIM.
If its any other code, you'll have to take it in to a repair center and have it reflashed.
This is a code your service provider can give to you. DO NOT try to guess it. Entering the PUK too many times incorrectly will permanently disable the SIM card and you will have to purchase a new one. Call your service provider to get the code.
