The PUK or 'Provider UnlocK code' is required when you enter the wrong phone unlock code a few times (normally 3 times): this PUK code normally comes with your SIM card documentationbut can be obtained from your cellular service provider help desk.
Do not attempt to enter the wrong PUK code more than a couple time as after a certain number of attempts (5 to 10 times), the SIM card will be permanently disabled and you will need to get a replacement SIM card.

you will need to ring your phone carrier. this usually happens when you have a sim pin code and have entered it wrong three times in a row.
you will need the imei number of the back of the sim card whenever you ring the sim carrier.
if it is the phone that is blocked then you will need the PUK code. you can usually find these by doing a simple google search. if not please provide the details of the phone and i will see what i can do
