Hi, the PUK Code is provided by Customer care of the network carrier of your Sim Card. Please contact Customer Care to get the PUK Code which will unlock the Sim Card.

If you are in a situation needing your puk code, then you have to contact your mobile service operators. If your line is presently blocked, preventing you from making calls, then use someone else'e line, of the same network as yours to call your mobile service operators. Tell them you lost your PUK codes and they would provide it for you. Please dont ever make the mistake of guessing the code to use on your phone. It could lead to a permanent blockage of your line.
I don't quite understand when you said "I've lost my sim card, how to get my PUK Code?", PUK code is specific for every sim cards, so if you have lost that sim card which was blocked, I don't think you will ever need the PUK code for that sim card. But if you mean that your sim card is blocked and you are being asked for PUK Code, please follow the instructions given HERE. This is one of my "how to's" about PUK Code issue.
pin codes one and two plus puk is not included in your phone's manual. It is printed at the back of your Sim Card pack. Some are shown while for the others, you need to scratch them first to reveal the codes.. :))
