Question about Samsung Cell Phones
PUK and PIN are two security features provided by the service provider. The defaults are 00000000 (eight zeros) and 12345. But you have already changed the PUK and forgotten it. The SIM has got locked and will have to be replaced by the service provider for a small charge to retaining the old phone number. So, please call up the help line of the service provider, even if from a friends phone and get a new SIM.
