Connection all depends on what type PA system you have. Here are the rules: Power this device from the SAME receptacle your mixer is powered by, even if it means running an extension cord. Use ONLY a balanced line from the mixer... either TRS cable or XLR cable.

How do I plug mixer into pa speakers, monitors, instruments

Without knowing what make and model of the gear you're looking to connect, I can't give you a definitive answer. However, in general, if you have a powered PA mixer, meaning it has both a mixer section and a power amp section in a single box, the main speaker outputs will be on the rear panel of the box and marked as such. There will also be a jack for monitors, but some mixers have an amp for monitors, some just have an output for the monitor mix, but not an amp for them, and on some you can switch the main outputs to feed either a Left and a Right speaker, or a front mono speaker and a montior speaker. Instruments will go into the individual channel inputs.

Problem is after a few minutes i lose all sond comming from my speakers

Not likely a grounding problem. Run a test: Turn on unit with NO sound into the mics and wait 5 minutes... now try the mics. If they start working for a few minutes, then the amp is being overdriven. These small systems can be overheated and a thermal protector shuts them down to protect them. A hidden problem is if the mics are in front of the speakers you MIGHT have ultrasonic (above hearing range) feedback that may be overheating the unit. Put your speakers nearer the audience and mics behind. If problem persists, exercise your warranty.

Using Boss FV 50H between output of Tascam M-216 mixer and church PA (line input of main mixer) with microphones on 1 channel and guitars on other. Problem:Quite a loud "pop" or...

The problem is you have an ILLEGAL connection based on proper electronic design. This pedal has UNBALANCED high impedance interconnects and the output of your mixer and the PA SHOULD be BALANCED low impedance interconnects. This makes the system subject to noise and you may even get static discharge noise. Also verify that the TASCAM and the PA system are ALL powered from the SAME receptacle or power conditioning module. This is a tough design problem to achieve what you want. Essentially you want to make the master gain of the TASCAM on a foot pedal as I understand it. There is one pedal that CLAIMS to have balanced in and out and if I had the schematic I could verify it... but I could not find the schematic in my searches... look at:
This one MIGHT do the job right. You MUST use TRS BALANCED cables, NOT TS, from the mixer to this and from this to the PA. Do research on BALANCED and UNBALANCED and TRS and TS and also XLR cables using google to understand the difference and importance of using balanced interconnections in your system. I would try to get one of these pedals on approval with the correct cables and test it.

When plugged into my churches pa system through a direct box the volume gets really loud then really quite and then sometimes normal. I'm using a lithium battery

Talk to your sound man... It is possible he has a compressor on the line. A compressor adjusts the volume dynamically and delays tweeking and releasing as a setting. Your guitar should NOT run through a compressor.

Hook up extra speakers

Most of these portable PA systems do NOT have amplifiers capable of driving extra speakers SAFELY without risk to the amp.

I would NOT attempt to drive more speakers without using another amp to drive those speakers.

Popping sound through sound system speakers

make sure that the trim (gain) isnt set too high on your keyboard's PA channel. it might just be clipping due to input levels that are too hot.
