PUK code and PIN code are being given by network providers only. If you are using a prepaid line then you should already have that one with you when you purchased the SIM card. I suggest to call the network provider if you are using a regular line. Just provide them you personal infromation and they will give you the code's that you need.

The phone asks for PUK code ( Personal Unblocking Key )if a wrong PIN code is entered repeatedly
PUK code is related to the SIM card you are using in your phone and not the phone itself. The code can be supplied only by the cellular service provider.
So, kindly call the customer support of your cellular service provider using another phone and they will provide you with the code free of charge after verifying the details.
Please note: Entering a wrong PUK code ten times in a row permanently disables the SIM card. So please be careful while entering the PUK code
