From your details i can observe the blower motor bearing worn out. or its side bushes in which there is small bearing that may be worn out. Replace it. Finally check for correct voltage too.
Maybe the fan is going bad. Open the case and check it to make sure it is tight by tightening the screws. Also make sure a cable or wire is not hitting the fan blades causing the noise.
Go into the Power Option in the Control Panel and set your Sleep or Hibernate options up higher or set them to NEVER.

Hi Lou, that's a good question. In sleep mode there should be absolutly no noise at all as the machine is asleep. If there is a noise then it is not asleep. Also, the chances of a hard drive in a laptop making a grinding noise is pretty slim so it is probably not the HD - if it was you would be having trouble using the Mac. It could be a fan that is still running to cool the machine down. How long does the noise go on for after you put it to sleep?
Don't worry thats not a big problem, you have a lot of dust deposited on the fan, thats why its making so noise.
Once you clean it, your fan will make very less noise, the way it did before.

If you are using a dimmer switch to operate the fan instead of a fan speed controller, that is the cause of the humming. If you turn down the speed to the lowest setting and the hum gets louder, then your controller needs to be replaced.
i would check the blades on the condenser fan motor and make sure they are not hitting anything if they are not, spin the fan see if the fan spin freely, it should, if it doesn't it could be the bearing in the motor are dry and going bad
actually fan really works even at sleep mode to exhaust heat, the problem is one of the three fan located at the left side if your facing the machine, is deffective a broken blade causes it to sound bad try to remove its connector, afer all you have two more working
