There are a wide variety of issues that this could be, and seeing as how you didn't clarify which console your using, I'm going to assume its the XBOX 360 because this is a wide ranged issue with that console. If your only having a problem with that particular disk then I think it's safe to assume that there is only an issue with that disk. i had this problem with my xbox and its due to either the laser or the magnet within the disc drive that holds the disc in place. the magnet may have come loose which mean u can either open the drive up to try and reposition it, or replace the drive all together. my suggestion would be to send the xbox to microsoft if under warranty or send it to a third party company to see if they can fix it. buying a new drive for your xbox involves some very complex firmware flashing which is not advised
I do believe its a problem with the disk. The disk may not be scratched or anything but it could possibly something internally wrong with it. I'd take the disk back and exchange it. Hope this helped! Have a great day!
