Often the disk being damaged by the drive is a result of a dirty spindle. ( the round part that holds unto the disk inside the machine.) this unit has a magnet on it that attracts and holds a plastic ring unto the top of the game disk. It helps clamp the disk and hold it in place. If the bottom section (which is usually a rubber ring) gets dirty it will not hold as it should and will slip causing reading errors and damage to the disk. You will need to open up the XBOX, open up the disk drive and then clean the spindle using a rubber reguvinator. If you do not have this available then you can use denatured alcohol. DO NOT USE RUBBING ALCOHOL! While you are inside this unit go ahead and clean the laser lens with denatured alcohol by gently cleaning it in a circular motion with a qtip and the alcohol. Again to not use rubbing alcohol, it will leave a film. Anyway, Check the laser and it's moving up and down. If it does not go all the way down or up you may need to replace the belt or clean it as well. If these things do not help you then you may have to replace the drive. There are several available for fair money on the net. JUST BEWARE OF WHERE YOU BUY IT!! There is no need for special software or programs. The drives are drop in and go. (by the way, to open up the unit, google for XBOX removals there are several sites with free instructions. get a couple compare them and you should do fine. Again no need to spend money on directions on how to do this. There are several good sites for free.)
Do note that the 360, unlike the original XBOX is not an easy thing to get into. It will take you some time to do it right. It can be done just be careful. I trained my 15 year old to do it, He tears them down for me before I repair them. I get 2 to 3 a week for repair.
Anyway, try these things and let me know how it goes.

SOURCE: keeps saying disc unreadable
"A temporary solution" If u got the new xbox experience and enough HDD space, install game to HDD and then play from HDD. You'll probably have to send back to Microsoft eventually. Though make sure its only COD4 thats giving trouble, buy some disc cleaner and see if that helps

i had this problem with my xbox and its due to either the laser or the magnet within the disc drive that holds the disc in place. the magnet may have come loose which mean u can either open the drive up to try and reposition it, or replace the drive all together. my suggestion would be to send the xbox to microsoft if under warranty or send it to a third party company to see if they can fix it. buying a new drive for your xbox involves some very complex firmware flashing which is not advised

Well the disc driver obviously works on most of your games except saints row 2. Saints row 2 is in mint condition well i have this problem too with my 360 it reads some games and it doesn't read my other ones well theirs a few ways one is by leaving the game inside of the console and shut it down wait like 5 seconds and turn it back on.
Another is to make sure the laser its self doesn't have dust on it.
And when you put in a disc listen to it as it spins if makes any weird scratching noise its probably the laser hitting the disc a lot of 360s disc have been getting scratched recently due to the fact in microsoft's original design of the disc drive the laser keeps on hitting the disc and scratching it so if thats the case you have to replace the disc drive.
If solutions don't help let me know i might be able to help you more.
