This means that the system wants you to reconnect the first player controller, usually in the 1 slot. It will do this if it thinks the controller is missing. I get this frequently on my xbox when the controller runs out of batteries.
Make sure the controller is plugged in all the way, charged and working properly!
Alternatively, you can power down the PlayStation and start it back up again.
The button pad is usually connected to the control board with a ribbon connector. They can become slightly loose, with vibration, over time. If you open the unit up, you will see where it is...it will be near the control panel. Detatch it and reconnect it. If it's a different system connecting the panel to the board, check any wire connections (disconnect and reconnect, ensuring they're tight), or solder joints for cracks, or cold solder joints (coudy looking joints, not shiny).
It could be a bad component, but since it's recurring, it's either an underengineered control panel, or a bad connection of some type.
Inspect it carefully, reconnecting any loose connections.

The buttons for reconnect are under the SD card reader door on the front of the console and under the battery door of the controller. They are both small red buttons, make sure you press them at the same time. Good Luck.
I have had the same problem in the past. The problem with mine was the controller wires were loose from being wrapped tightly around the controller while not in use.
Try pugging in a new controller or one you know does not have issues.
