That is because you or someone else has tried to enter the PIN-code on the phone more than three times. This means that you will have to enter something called a PUK-code, which you probably got together with the SIM when you got it. But if you cannot find it, your best (and only) option is to contact your provider (like ATT or Verizon) for help.
