It is called the 3 button reset or hard reset. Below are the steps that should be taken to successfully unlock your phone.
Power off phone. Press and hold Green, * and 3 and keep them held whilst powering on and for a few seconds after, if kept pressed this formats without asking for Security Code confirmation.
