You have ceramic body type fuse, you need an Ohm meter to test it.
You can test it using flash light, you have to out the fuse inseries with with the lamp to see if it will light up.

Or www.digikey.com, just make sure to use caps with low ESR, 105c, high ripple current and long life rating such as PANASONIC FM or FC series.
