When you are receiving a call use the up/down or left/right rocker (the squarish thingy around the circular button in the middle) to increase/decrease the volume. This also works for the speaker volume if you have the speaker on.

During the call press UP on the square button in the centre of the phone(when not on a call this is the default short-cut for your camera)

This can be due to the phone speaker. It might be faulty or in need for replacement. You can try to visit a phone service center to have it check or if you are capable then you can check it yourself.
