You can obtain your PUK code from AT&T by following the instruction on the following site http://www.wireless.att.com/answer-center/main.jsp?t=solutionTab&solutionId=KB617 This is assuming you are using AT&T, if you have a different carrier the instruction should be very similar. Hope this helps!
