SOURCE: PUK code for lg KP 500
You need to call the phone company that sold you the phone card to unlock the phone.
PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the puk code from a database for free.
In your case you need to contact your SIM card phone provider technical support to unlock the SIM.
The PIN and PUK are related to the SIM card, not to the phone.
If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.

Call to customer care using any other mobile phone which uses the same service provider. They will ask some details about you to make sure you are the owner of the SIM card. Questions might include your billing address, email ID, numbers you recently called or messaged etc. They might even ask you number printed on the SIM card, so be prepared with all details before calling them. Once you get the PUK code, enter it into the phone & your phone will return to normal. Hope I helped you.

Testimonial: "Thankyou..will call the provider...much appreciated advice.."
PUK CODE: This is usually a problem associated with a LOCKED SIM CARD. This can sometimes happen when you enter your PIN incorrectly too many times. I would recommend going to a retail location of your service provider (ex. AT&T Store) and ask for a SIM CARD SWAP. This service is free for most providers. If you are charged for a SIM swap, tell the person helping you that SIM swaps do not cost any money and that they are free and that is what you were told over the phone.
OTHERWISE YOU CAN TRY THIS:
PHONE UNLOCK (Not related to PUK Lock):
There are websites online that provide unlocking services for a fee. If you google "phone unlock service" you will see many options.
There is also another way to go.
You can call yout service provider (ex. ATT, T-Mobile) or the company that branded the phone (the company that the phone is locked to) and tell them that you would like the unlock code for your phone. They will probably ask WHY you want the unlock code. Tell them that you will be going out of the country and need the phone unlocked in order to use a pre-paid SIM card in the country of destination. They will then get you the unlock code typically within three days. Hope this helps.

PUK code can be given only by your service provider which is specific for each and every mobile number.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them.
Go to the nearest service outlet of your service provider they can also help you.
There is no other go than the above procedure only your service provider cangive it nobody else in this world nor any software.
Go to the below link and you can confirm that the above procedure works and there is a testimonial given by the customer too. http://www.fixya.com/support/t3582685-phome_lg_ke670_put_security_code_phone
