Installed a Graphics card EVGA 6200 AGP 8X in my PC HP Media Center PC m370n now keeps showing failure notice

Did you install the drivers for the Graphics Card before you plugged the card into the computer? If not, remove the graphics card and reboot using your old video connection, install the driver then install the card and plug your monitor into the newly installed card. If you still have an issue try booting into safe mode.

Nvidia boot agent ?

I would state use the Restoration Disk, and perform a Repair. (Non-Destructive Installation)
This will repair the Nvidia settings for YOUR computer.

But more than likely Acer did not provide a Restoration Disk, and it is my understanding it may be hard to get a hold of. (Acer Support)

This makes me think you have installed a new Nvidia graphics card, since specifications indicate it comes with Integrated Graphics that is Nvidia GeForce 7050.

{Integrated Graphics is also referred as OnBoard Graphics.
ON the motherBOARD. The graphics chipset (GPU) is soldered to the motherboard, and Not to a removable graphics adapter card,

http://en.wikipedia.org/wiki/GPU

IF SO, the software (Installation disk) is installed FIRST, then the graphics card is physically installed.

IF not you get a driver conflict.

When a device is installed on a computer, (Such as a graphics card), Windows 'breaks it's neck' to find matching suitable drivers.

If THE drivers are not there, Windows will match up one of it's THOUSANDS of generic drivers.
This won't do.

Windows: "Which drivers do I use? These or THESE?"

[Drivers are small pieces of software, that allow the Operating System to 'communicate' with a device ]

Is this what you did? Install the graphics card, THEN use the graphics card's installation disk?

I have a Dell Optiplex 960 in which the motherboard had to be replaced. When I try to install nvida quadro 420 the machine reboots and will not go into windows

Could be a bad power supply. If the power supply can not handle the nvida quadro 420, the computer would be forced to restart. You computer doesn't essential need a graphics card to work. The mother board has one embedded so your desktop can run without it. My suggestion is to take the card out, reboot it, and find the correct drivers online before putting the card back into your motherboard.

New Video Card fan sometimes won't spin (in new self built pc)

I would RMA the card if the fan refused to come on. When it is not on and you begin testing the card, does the fan come on at that point? How do you make the fan come on other than turning on your computer? If the fan isn't on to begin with, but comes on when you start playing a graphics intensive game I would not RMA the card unless Nvidia support suggests it.
