Water coming out from under lid there is two thing's to check. Fill tube from fill valve line may have come out of stand pipe and spraying under lid. And the next thing to check is the top of fill vlave maybe leaking and sparying water from it causing water to come out.

The toilet tank has 2 dips in the back wall and the lid does not cover them. it is allowing moisture to get on the wall and cause mold. should those dips be there or?

yes. Why so much moisture? There shouldn't be any unless you have hot water filling the tank or a spray of water shooting out. Check it out.

I've installed a lot of Toto toilets and have had few leaks but, not from the one piece toilets. It might be a problem toilet but, I would be sure the drain is not blocked causing a small back up after the flush.I have seen this happen many times. The toilet can be pulled and a 5gallon bucket of water can be poured down the drain to test this before replacing the toilet. also be sure water is not spraying on the top of the tank lid and leaking down the back of the toilet. If you flush the toilet with the lid off it should be easy to see if that the problem. hope this helps.

Water squirts from tank while filling

Take lid off and check that the fill tube is in place. If so flush with lid off to see were the water is squirting from, be prepared to clean up water...If you can, do a slight flush. If there is water coming from any where but the bottom of fill valve and the fill tube you may need to replace the fill valve. In my experience, the fill tube is dissconected.

Basement toilet drains slow sometimes ,other times doesnt want to go down all the way. seems as if there is no pressure to remove waste

if the toilet is old or not cleaned proper, the water jets that shoot the water into the bowl might be clogged. If the jets are clogged with moldor hard water build up the toilet will not flush proper. If you can take a 3 gallon bucket of water and pour straight into the bowl and it flushes fine you have a water pressure problem as described above. the problem also might be that your water inlet system is not letting in enough water to fill the tank. If this is the problem you need to replace the water inlet mechanisim, such as a fluid master. If you use a blue water device that hangs on the side of the tank in the back of the toilet you might have a peice of it under the flapper that takes the water to the bowl when being flushed. Check under the flapper if you put cleaners in the tank.

The toilet recycles every ten minutes

You got a bad seal for sure.  Even though it looks like a good seal.  Open up the lid and see if you got any little bubbles floating up from around the seal or listen for a slow water drain around the seal.  Go to Lowes and tell them you need a Korky flapper.  Their the best thing since sliced bread and peanut butter.

2 Toto Guinevere toilet levers stay up after flush - wastes water

Often the chain or cord in the toilet tank has caught on something. Only lift the tank lid when water has stopped running. When replacing the tank cover, check if vertical water pipe has to sit under the lid just so to catch the vertical discharge, if so designed. Or perhaps the rubber stopper has deteriorated. Good luck
