If you use the same sim card, this PUK will still be there. You must contact your provider for the puk code. If you still have "attempts" remaining you can save the sim card. If it says PUK Locked...then u need a new sim card.
The PUK (Personal Unblocking Key) and PUK2 are used to unlock the PIN and PIN 2 codes respectively if your SIM card is blocked. It means that the problem here is your sim card and not your phone. You don't need to get a new phone just replace your sim card.
