Does this problem also happen when you try to make a copy using the Copy button on the printer operator panel? You can try to reset your printer to factory defaults. Follow the instructions in this article on How to Restore the Factory Defaults on the Lexmark X6100 Series.

IF YOU ARE ATECH OPEN THE TOP COVER UNDER TE DOCUMENT COVER (4 scrowes take out the glass and wip the mirrors the lamp reflectors and fix them back
I had this problem too - then downloaded new drivers and software here:

You need to re-install the drivers, then use the MP Toolbox software, and play with the "scan mode"
