Sounds like the clutch is not releasing properly. If it grinds when trying to delect reverse this will be the case. It may be a hydrolic fault or the clutch itself. Check to see if the slave cylinder rod is actuating when the pedal is depressed.Check for loss of fluid.

I was towing a heavy load with my 94 4runner. i put it in 4wd, heard a strange moan sound, very quickly took it out of 4wd. now i have no reverse

Not sure if the 4runner requires you to stop before engaging 4wd or not. Need some more information on your vehicle, auto/manual, speed when you engaged 4wd.

1st, 2nd gear and reverse will not engage on my 4 speed manual transmission. what is the problem and how should i go about getting it fixed?

Sounds like the transmission is worn out. It could be rebuilt or call around to the junk yards and see if any has a good clean used trans. If you get a junk yard one change the fluid before you drive the truck.

Transmission not going into gear

I see this is kind of a stale post, but i'll try anyway. First, check your clutch fluid reservoir. If it's full, you may have air in the line, a bad master cylinder, or slave cylinder, or your clutch is worn out.

Vibrating Doors

You may need new motor mounts...if they are worn out this would allow additonal vibration from the engine to transfer to the vehicle body & doors . If it is 4WD then also check underneath for the center driveshaft mount. Higher mileage cars tend to have the mounts lose there ability to dampen engine movement upon engagement.

Any shop should be able to push on the brake and engage the transmission in drive or reverse and watch the whole vibration event happen.

Then it becomes a determination of which mounts to replace.
