You should have the Personal Unblocking Key (PUK) number in with the instructions or the leaflets that came with the phone. If not try ringing the customer services to whom your phone is set to. i.e. Orange, T-Mobile, O2.etc. Tell them your details and they might be able to help. Make sure you have your mobile with you when you ring them. If they can't fix your problem, then you'll have to take it to someone who unblocks phones. There are plenty about (normally pc repair shops do it). If not look down the high street next time you visit.
