Pda locks up. After hitting

pda locks up. After hitting reset, it will not go past time and date. It seems to work ok when pluged into 110 charger and usb cable.

When i pluged charger in to phone my phone asking phone lock code and i tried out many option and code not work so any other to reset a phone or via usb?
