Floppy disk drives (and disks) are not that reliable and usually fail, especially on older computers. A test would be to try a different floppy, also try formatting a different floppy. Can you read the problem floppy from a different computer?

Try this. HP's are notorious for having these. Since they were shipped(could have been shaking, etc.); this might work.
Take the side off of your tower. locate your CMOS battery on the motherboard.(looks like a watch battery;about the size of a nickel. Then plug up your computer(with the CMOS battery out), power on for at least 10 seconds. This will reset your system back to default settings. Now; re-install the CMOS battery, and power back on.
See if that fixes it...

If pressing F11 ("system recovery") does nothing, then your disk-drive, which contains a "system recovery partition" is inaccessible,
i.e., your disk-drive has failed.

You'll need to buy a new disk-drive, install it, and use the original "System Restore" set of CD/DVD disks to reinstall Windows onto the new disk-drive.

Elder parents have a 1 1/2 yr old HP Pavillion Series 4600. They are getting a black screen that reads "Disc Boot Failure Code HP-BIOHD2. Can you help me?

That means that the BIOS of the motherboard can't detect the hard disk = it has died. Replace the hard disk and reinstall.

Xp install disk cant find any hard drives on my hp pavillion dv 5000. it was given to me without any install disks

You will have to delete the partitions of the hard drive before using the xp disk because if there is any partitions formatted as EXT (non dos) the xp disk will not read them. You can download a dos disk of the internet to do this. Once you have used the fdisk command which you can use to re-partition your hdd you should be able to install xp no problem.
