Re: it a nokia n95
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.

PUK means "personal unlocking key"
. The PUK code is asked if you enter a wrong pin 3 times. You
need to call your phone service company, they can retrieve the puk code
from a database for free. In your case you need to contact your
SIM card phone provider technical support to unlock the SIM. The PIN
and PUK are related to the SIM card, not to the phone. If you replace
the SIM card with a different one, then the phone should not ask for
the PUK anymore. Your rating is
highly appreciated on this matter.
PUK is not your phones code. its the sim code. if you enter three times wrong PIN number then the sim asks u for a PUK to unlock the sim card. if u enter 10 times wrong PUK code then your sim will get corrupted and u cant be use that sim anymore. to solve this problem. you have to contact the sim vendor or customer care to retrieve the code from them. u have to give 16 digit sim no: while they ask for conformation. after getting the PUK no. keep that Number in safe place for future use.....
Access codes from page 132 in your manual: Q: What is my password for the lock, PIN, or PUK codes? A: The default lock code is 12345. If you forget or lose the lock code, contact your device dealer. If you forget or lose a PIN or PUK code, or if you have not received such a code, contact your wireless service provider. For information about passwords, contact your access point provider, for example, a commercial Internet service provider (ISP) or wireless service provider.
