The hole where the needle is supposed to go appeared to be too small for the needle to be entered with flat side in the back as shown by only two views on Page 31. Please help me.

Best advice is to take the sewing machine to a repair center, where you can buy a new needle and and ask the service person to install for you. If you bring your own needle, you may get charged for the installation. On the other hand, if you buy a needle from them, you might get a free installation.
Try that.

My brother ls-2125i sewing machine is making loops and loose threads and still does after being cleaned, oiled, and threaded again. What could be the problem, and how can it be solved?

change the needle
Correct needle insertion is mandatory for proper use of your sewing machine.
Raise the needle bar to the highest point, then loosen the needle clamp screw to remove the old needle.
Place the new needle in the needle clamp with the flat side of the needle facing the back, (round needle, the groove in the needle should face away from the shaft).
Push the needle all the way up as far as it will go. Then tighten the needle clamp screw.

adjust the top thread tension one number or letter at a time until
the stitch looks normal

The needles are changed by inserted the hex wrench into what looks to be holes on the front and left side where the needles go into the machine.I'm not sure what size the hex wrench is but it's pretty small. From the manual,
"Removing Needle: Unplug the machine. Turn the handwheel counterclockwise to raise the needle bar to its highest position. Loosen the needle clamp with the allen wrench and pull the needle out." and
"Inserting needle: When the neddle bar is at the highest position, insert a new needle in the needle holder, making sure that the groove of the needle faces you. Push the needle as far up as it will go and tighten the needle clamp screw." "Removing Needle: Unplug the machine. Turn the handwheel counterclockwise to raise the needle bar to its highest position. Loosen the needle clamp with the allen wrench and pull the needle out."

The machine takes DB x 1, or 1738 needles, and a size 14 needle is recommended.

Hope this helps!
