hi friend the only way to resolve your problem is goto to your network provider because they are the one responsible of your PUK code,some of the provider give the PUK code to there subscribers,it is included in the simpack package....
When your phone ask for a PUK code,it means you enter wrong PIN code for several times,and you need PUK code to retrieve your PIN code further.

hi! nothing to worry! i think that ur sim locked that is due to entering wrong unlock code for more than three attempt..! now move to solve that problem! try to get ur sim card pouch that you got with at the time of sim purchase! in that puk code is printed on it! simply enter that code ur phone unlocked! if wont get puk cod or missing that sim pouch ! dont worry! simply call ur mobile network operator by using other num plz note the same operator number is used for calling( i.e need code for aircel num u need call another aircel num to get code) then call to customer care and ask puk for ur number before that u make note that the number quoted on the simcard..! then collect ur puk number! kindly save that number for future use! ! thats it now ur phone unlocked.! When you entered a wrong pin number three times the phone asks you for a PUK number. The PUK code is generally eight digit number. This PUK number is an unlocking code which helps you to unlock the phone and use it. If you enter a wrong PUK number 10 times than you simcard gets rejected. Then you will have to buy a new simcard. It might happen that your phone might ask you for PUK2 after you enter your PUK number. You can find the PUK code is in your SIM card documentation Starter Pack. If you do not have this then contact your phone service provider. Your cell phone service provider can give you the PUK code once they have confirmed your identity to prevent any stolen SIM access.

You will need your IMEI number from your phone. It will be under the battery or if you can...Type. *#06# into the phone and this should tell you your IMEI number.

You should have read the User guide for the phone. There it clearly says that PUK and PIN are given by the service provider. The default PUK is 00000000 (eight zeros) and PIN is 12345. If they are supplied wrong three or more times the SIM gets locked and has to be replaced. Your number remains the same but you have to pay for the SIM. Please contact the helpline of your service provider and ask for a new SIM as you seem to have given wrong PUK more than three times.

The PUK (pin unlock key) code is usually printed on the Back of the SIM pack check it out and if it isn't there then u need to call your carrier and they will give to to you after verification. Make sure not to enter the PUK code your self after 10 incorrect attempts the Sim will be permanently blocked and then u'll need to get a new one!
