The only way to get the PUK code is to call the service provider(not the phone maker) and give them the number that is on the sim card,They will give you the code needed to unlock it.So just have the sim card in hand to read off the number when they ask.

Just call your provider and tell them you need a PUK code, they will be more than happy to give it to you.
you may also get it from the company website or go into the provider store.

There isn't - you have to call AT&T in order to get it. There is an after hours line that your local store can use if regular customer care is late.
