The heatsink keeps the CPU from getting too hot and the fan cools the heatsink. These fans are temperature controlled by the BIOS firmware. When there are problems that need to be fixed(updated) Dell creates a new BIOS flash firmware which can be found on their website. Make sure your BIOS version is the latest. You can see which version you have while the laptop is booting, press F2 to get into that BIOS. My D600 ran hotter than I expected until I updated the BIOS firmware.
