Question about Cell Phones
This is hard to answer without knowing the make and model of your phone. But, there is a simple workaround. Most Micro SD cards bought today come with an adapter that allows it to act like an SD card. If you have this, put it into any computer and format it to the file system type used by your phone.

This is a problem due to the memory card IC.The IC has to be replaced .Better contact service centre.If you are familiar with Mobile Circuits you can follow the instruction in the following diagarm.this will solve the problem

SOURCE: I PURCHASE 4GB MEMORY CARD
1. Insert the memory card in a card reader.
2. Connect it to a PC or a laptop.
3. Go to My Computer > Removable Disk
4. Right click on the Removable Disk icon and click on format.
5. This shall format your memory card.
6. Run an anti-virus scan on it in order to fix any possible bugs.
