Open iTunes and click on the 'ringtones'tab.
Make sure the Download all selection is chosen, or check the box next to all the ringtones.
Click sync, and the ringtones should sync to your phone.

I succesffully made 3 ringtones, working fine, assigned to contacts, none over 25 seconds long. Went to make the 4 one using exact same method as previous successful 3 tones. When 4th one made, the other three disappeared even tho they still show on my phone. Frustrated.

I can only get three ring tones on my Iphone, even though all of the ring tones show up under my device in Itunes. Please help. Turning off the phone and restarting or re-synching doesn't work.

Ringtones that exceed 0:30 (30 seconds) that are in your library will not sync with your device. If these ringtones that won't sync are over 30 seconds you will have to find new ones that are less then 30 seconds.
