I only know 2 solutions to this problem,
Solution number 1: Your disk is not working, There may be scratches or smears. If there are scratches, you need to find someone who can buff out the scratches. If there is a smear, all you have to do is run the disk on some water (Hot, Warm, or Cold) then rub it with something like your shirt.
Solution number 2: Your disk slot is broken, what i do with mine is hit the top while it is reading the disk, then it works.
If none of those work, Call an Expert.
