The PUK that comes up on your mobile is simply asking you to enter you PUK code in order for you to be able to access your phone. The PUK simply means Personal Unblocking Key.
When wrong codes are being entered as security code on the sim card, the sim gets locked for security reasons and required you to enter the PUK in it in order to unlock the phone.
The PUK is always in the starter pack or sim card kit. When you purchased your sim , it came in a pack, the pack consist of a user manual, a card that the sim was attached to (the card you break off the sim from with some bar codes on it) all in a pack.
On the Card you broke the sim card out of, you will see your mobile number and your PUK code, in most cases it used to be 8 digits, and is some cases lesser or more, depending on the sim card service provider.
Get the pack You will see the PUK code enter it, after entering it, it will prompt you to enter a new pin, enter it, then it will finally ask you to re enter the pin, after re entering it, you will have access to the sim card once again.
