Hi the PUK code is the security code for the sim card and not any code to do with the phone, all you need to do to get the PUK code is call your service provider.
your service provider wont charge for giving you the code as the code belongs to you, all you need to tell them is your phone number or if you dont know that then the number printed on the sim card, they will then provide you with the 10digit PUK code for free over the phone.
this will then allow you to access the sim card.
The only place you can get the PUK code from is your service provider as their is no where else that keeps these codes, if you still have the card the sim card was attatched to then it will be written on their but if you no longer have this then the only way your going to get the code is by calling your service provider, they wont charge you for this as the code belongs to you,
you do have another option open to you though, if you dont need that sim card then you can go out and get another sim card and when you get given a new sim card you will get given the unique PUK code it will be written on the card the sim card is attatched to.

PUK is a pin unblock code - your provider can get it for you.
*#06# is NOT your PUK it is your IMEI number
Since the PUK is sim card specific it will not be on the box or anywhere with the phone you MUST call your provider for it.
If you enter the PUK incorrectly 5 times you will burn out your sim and need to purchase a new sim.

Hi,the only way you can get your p.u.k number back is by contacting your service provider,they will give it to you free of charge.but you will be asked some legal questions so as to confirm that you are the owner of the phone.nobody aside from your service provider can give you your p.u.k code.
