PUK codes are sim card dependent. If your under a subscription plan, you can ask you provider of the PUK code. If you are using a prepaid sim, the PUK code is on the sim pack. It is on the card where you took your sim out. If you don't have the sim pack anymore, unfortunately there is no way that your SIM can be unblocked. You would need to buy a new sim.
