By default the lock code on CDMA phones is the last 4 digits of the phone number so if you number was 555-555-0909 your lock code will be 0909 unless other wise changed, other than that the only way to get the code is trial and error.
Try 1234 if it's the 4-digit lock, or 000000 if it's the 6-digit; these are the factory defaults.
If your service provider is verizon, then the 4-digit code is the last 4 digits of your phone number.
If none of that works, then you're advised to contact your service provider.
