request for a PUK code ("Personal Unlocking Key") will appear on your
phone if your SIM card PIN code is entered 3 consecutive times
Your SIM card will then become "PUKed" or Blocked. This is a security measure to protect your service.
You need to call the Customer Service department of your cell phone provider and they will give you the PUK code which will unblock your SIM card.

For this, you should contact Customer Care of your Mobile Service Operator (Network Provider).

your only option is to call your phone service provider, no it doesn't come with the phone and the phone never has its own puk code becouse the puk code is about the sim card.
if you put in another sim card the phone will work just fine, its the simcard 3 times pin code wrong puk code enabled pin blocked, call service provider to get it and entery instructions.
