You need to contact Orange, is the only way to get the PUK. Here is general Orange website. At the top there is a box that says group sites, select your country in the box and then Sign up to manage your account online, so that you can visualize page with customer service number.
Once you get the number phone them, they will ask phone number and model number and give out your puk code. If you post here a new request with your country we can find the number straight away. Alternatively put the following sentence in google, you should find the number the same

Hi the PUK code is the security code for the sim card and not any code to do with the phone, all you need to do to get the PUK code is call your service provider. your service provider wont charge for giving you the code as the code belongs to you, all you need to tell them is your phone number or if you dont know that then the number printed on the sim card, they will then provide you with the 10digit PUK code for free over the phone. this will then allow you to access the sim card.
The only place you can get the PUK code from is your service provider as their is no where else that keeps these codes, if you still have the card the sim card was attatched to then it will be written on their but if you no longer have this then the only way your going to get the code is by calling your service provider, they wont charge you for this as the code belongs to you,
you do have another option open to you though, if you dont need that sim card then you can go out and get another sim card and when you get given a new sim card you will get given the unique PUK code it will be written on the card the sim card is attatched to.

this code belongs to you so dont pay anyone to give you the code ok, so the best option would be to call your service provider over the phone and ask them for the PUK code, they will ask you for the phone number if you know it or if not they will ask you for the number written on the sim card, once they have this they will then be able to give you the code, this code will only work with the sim thats asking for the code, if you enter the wrong code more than a few times it blocks the sim card and it can no longer be used,
As ginko correctly said above, The PUK is exclusive to your sim card, not your actual phone.
Easiest option is to obtain the PUK from Orange customer services. Call 450 from anothe Orange handset, or 07973 100450 from a landline.
Make a note of your PUK when you get it, because it will always remain the same (for that sim card)
PIN CODE is SIM dependenet and not phone dependent
PUK code in PIN UNLOCKING CODE, which is required when you give PIN number wrongly for 3 times, This code is OPERATOR specific and not phone specific and you have to call you operator and get PUK if PIN is blocked...
if you provide PUK code wrongly for 10 times, you sim may become useless, then you have to buy new SIM

ahhh this happened to me last night you need to call your service provider and give them your account info and its a breeze they give you the password in a matter of minutes but don't keep trying and use up all 10 chances or it Fry's your sim card
PUK stands for Personal Unblocking Key, and it is a unique
identificative code to prevent theft, or unauthorized users from using
your SIM. Even if it comes with the phone, PUK code is associated to SIM card, not to the phone.
When you input wrong SIM card pin three times , the phone will ask the PUK code.
Usually PUK is printed on a sticker inside phone box.
If you have lost your PUK code you will need to call the company that
provides you the phone service when you bought the phone (eg.
Vodaphone, Orange, 3, Telecom etc.).
The only alternative is going to a phone shop, they will call the phone company , get PUK and unlock your SIM.
Some phones have also an unlock code, but this is not called PUK, if phone asks for PUK you need to call phone company. There is no way to get PUK straight from the phone. If phone is locked with Phone Lock Code go to a local phone shop, they will connect it to a computer and get the code.
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
